Přístupy k léčbě obézního diabetika (kombinace antidiabetik s antiobezitiky)

Léčba obézních diabetiků je primárně režimová (dieta a fyzická aktivita). Nestačí-li režimová opatření, je perorálním antidiabetikem volby metformin. Dle potřeby přidáme jediný vhodný sulfonylureový preparát pro obézní glimepirid, event. dále inzulinové senzitizéry, akarbóza, glinidy či inzulin. V poslední době se však ukázalo, že ještě důležitější než kompenzace diabetu je u diabetiků adekvátní kontrola krevního tlaku a léčba hypolipidemiky. Léčba antiobezitiky sibutraminem či orlistatem je plně indikována ve všech fázích rozvoje diabetu 2. typu. Vhodná je kombinace s metforminem, deriváty sulfonylurey i s inzulinem. Antiobezitika jsou u nás hrazena při BMI nad 35 a splnění dalších podmínek. Z léčby mohou také významně profitovat i diabetici s BMI nižším než 35 či normálním.

Most important treatment of obese type 2 diabetics are life style changes (diet and physical activity). Where these are not sufficient, the first choise antidiabetic drug is metformine. If necessary the only suitable sulphonylurea for obese is glimepiride, if not effective the combination therapy using insulin sensitisers, acarbose, glinids or insulin can be used. It was shown in last years that even more effective than diabetes compensation is antihypertensive and hypolipdemic treatment. The therapy using antiobesity drugs sibutramine or orlistat is indicated in all phases of type2 diabetes evolution. Very effective is the combination with metformin, sulphonylureasand insulin. Antiobesity drugs are covered by medical insurance only in diabetic with BMI over 35 fullfilling some othert conditions, they are very effective also in pateintes with BMI 35 or with normal BMI.
